چکیده

We reviewed the pathophysiology of gout in this review article. Gout occurs as a response to monosodium urate crystal (MSU) that is present in joints, bones and soft tissue. Hyperuricemia in gout is mainly due to the decreased renal clearance of uric acid and is considered to be the cause of MSU formation. However, hyperuricemia alone does not explain the development of gout. Acute gout attack is due to the response to MSU, which is derived by innate immunity. Tophi are deposits of MSU and cell debris surrounded by granulomatous inflammation. (Korean J Med 2011;80:251-254)

Advances and unmet needs in gout

Gout is a common disorder characterized by chronic hyperuricemia and recurrent attacks of acute inflammatory arthritis, affecting approximately 1–3% of the population. Recently, gout has received increased attention partly owing to the development of new therapies (including the first approved gout treatment in more than 40 years) and an improved understanding of its pathophysiology. Recent res...

Refractory Gout Attack

Refractory gout attack is an uncommon problem, since gout flares are usually self-limited. This clinical condition is characterized by serum uric acid higher than 6 mg/Dl or continuous manifestations of recurrent flares, chronic arthritis, and increased tophi. We report in this paper a 69-year-old man with a polyarticular and protracted gout attack, despite usual treatment and low urate sera le...

Imaging Appearances in Gout

Gout is an ancient disease. Last decade has brought about significant advancement in imaging technology and real scientific growth in the understanding of the pathophysiology of gout, leading to the availability of multiple effective noninvasive diagnostic imaging options for gout and treatment options fighting inflammation and controlling urate levels. Despite this, gout is still being sub-opt...

Gout Transitions from Medieval Times into the 21 Century

Gout is the most treatable arthritis in the Western World—the pathophysiology of which is related to uric acid metabolism and there are effective medications available to treat both acute arthritis and chronic hyperuricemia. Despite this many patients continue to suffer from tophaceous gout with major detrimental effects on patient-reported outcomes and substantial economic impact. Poor adheren...
